License Retirement
Dentists who no longer wish to practice in Kentucky should formally retire their license rather than simply allow it to expire. License retirement not only informs the Board of your intentions, it also makes it easier to reinstate your license if you ever decide to return to active practice in Kentucky. You can even select the date you want for your retirement to take effect. To retire a license, simply fill out and submit the License Retirement Form.
License Reinstatement
Dentists who have previously held a license in good standing in the Commonwealth of Kentucky may request reinstatement of their license without repeating the entire license application process. See 201 KAR 8:533 for complete licensing rules.
Active CPR, BLS, or ACLS
Submit to digital fingerprint scan via IdentoGO Universal Enrollment Platform using service code 27GJYG.
Actively practiced within the two years preceding the filing of the application or complete a board-approved CE plan.
Official verification of any state licenses held since Kentucky licensure, received by the Board directly from the licensing agency
Former licensees who have not practiced in the two years preceding the reinstatement request must fulfill a continuing education
requirement. Applicants may submit any completed or proposed CE for the Board's consideration as part of their application.
Otherwise, the Board office will contact the applicant to develop a plan to meet this requirement.
